OIR-B1-1802 and the 4-point worksheet

Many Florida inspectors sell a wind-mitigation inspection and a 4-point on the same trip. They are not the same form, they are not interchangeable, and finishing both in one login does not mean a carrier has to accept either file.

The 1802 is a published OIR form

OIR-B1-1802 Rev. 04/26 is the Uniform Mitigation Verification Inspection Form. It has numbered sections, a photo/document rule on questions 2–9, and an inspector signature. FLOIR publishes it. What the form is is the locator page. WindMit Writer copies those sections into a job sheet; it does not replace the official PDF as the definition of the form.

The 4-point is not a statewide statutory form

Florida does not publish one official “4-point inspection form” the way OIR publishes 1802. In the field, a 4-point usually means a look at four systems — roof, electrical, HVAC, and plumbing — with ages, visible condition, and photos of deficiencies. Carriers (including Citizens) publish their own instructions and sample packets from time to time. Those instructions change. They are not our worksheet.

WindMit Writer emits our 4-point worksheet covering those four systems: type, age or last update, what you could see, hazards, and photo slots (dwelling sides, roof slopes, water heater / exposed plumbing, open main panel, HVAC with a dated plate, each noted deficiency). We do not title it “Citizens 4-Point.” We do not claim it satisfies a specific underwriting desk. If a carrier wants their own PDF, use theirs.

Same header, different questions

Property address, year built, and contact fields should match on both PDFs from the same visit. Roof covering on the 1802 is not a substitute for “roof system age and visible leaks” on the 4-point. Opening protection on the 1802 is not a substitute for an electrical-panel photo. A consistent job sheet exists so those two packets share a header and still keep their own questions.

Still not a yes from a carrier
